Covid Vaccination Status - All participants
Huron Perth Public Health, the Middlesex-London Health Unit and Southwestern Public Health announced additional proof of vaccination requirements on October 6th for recreational facilities used for organized sports. Proof of COVID-19 vaccination will now be required for anyone over the age of 12 who enters an indoor area of a sports or recreational fitness facility to participate in, coach, officiate, or watch organized sport. The requirement will come into effect on October 31st. All staff, contractors, eligible players, players family members, coaching staff, board members and volunteers must be fully vaccinated for COVID-19 to participate in hockey activities (on or off the ice) unless an OWHA approved exemption has been granted. All participants must be fully vaccinated for COVID-19 by October 31, 2021, which must include the two-week period required to be considered a complete series (second dose prior to October 17, 2021). A separate form must be completed for all individuals in your family who participate in Aces activities as a player, coach, volunteer regardless of date of birth. Please have participants date of birth and proof of vaccination (as applicable) available prior to starting the form. This information will be kept confidential and shared only with your team as required. This form must be completed by October 17th. Failure to provide proof of vaccination will result in that participant to no longer be able to participate in any activities until such time as completed vaccination results have been provided.

Vaccination Status

In order to participate all particpants must be fully vaccinated. "Fully vaccinated" refers to an individual that has received the required doses of a Health Canada or World Health Organization approved COVID- 19 vaccine and 14 days have passed since the last dose was administered.
